1. Information on how to enter and prize/s form part of these conditions.  By participating, entrants agree to be bound by these conditions.  Entries must comply with these conditions to be valid.
2. Entry is open to all New Zealand residents except employees and immediate families of Green Cross Health, its associated companies and  any agencies involved in this promotion.
3. This promotion commences 15/01/2021 and will run as a continual promotion. During each promotion period customers that provide feedback receive automatic entry into the promotion.
4. Entries are limited to one feedback response per day.
5. Purchase any product on https://www.lifepharmacy.co.nz/ during the promotional period and provide feedback at www.feedback.fyi within the period set out in point 3 above.
6. You must keep your proof of purchase to use the discount code. Photo ID may be required.
7. Each feedback gives the entrant 10% off their next purchase on https://www.lifepharmacy.co.nz/
8. Entrants will receive a unique code after submitting feedback online at www.feedback.fyi
9. The person whose email address is used to submit the feedback is deemed the entrant should any dispute arise. You may be required to present proof of purchase in order to use the discount code.
