1. Entry to the promotion is deemed acceptance of these Terms & Conditions.
2. Entry is open to all New Zealand residents except employees and immediate families of Amazon & GSM its associated companies and any agencies involved in this promotion.
3. This Competition commences at 9am on 4th of June 2014 and ends at midnight on 31st December 2015.
4. Entries are limited to one entry per unique code per customer.
5. To enter simply visit www.feedbackhq.net enter the unique code printed on the form at the front & your rating number with 1,2,3,4,5 and any additional comments you may have.
6. Your entry goes in the draw to win a $500 Amazon VIP Shopping Experience. The winner will be drawn on the 1st of each month.
7. Prizes are not transferable, exchangeable or negotiable and are not redeemable for cash.
8. The winner will be notified within 2 days of the draw & contacted via email and a call to the mobile phone number that is listed on the entry form on the website www.feedbackhq.net. Should contact not be able to be made on this mobile phone number within 5 working days, the winner forfeits the prize and the prize will be re-drawn.
9. All entrants may receive future communication from Amazon. If respondents wish to opt out of future communication Amazon respect that and will automatically remove their contact details from all future communication.
10.All decisions are made by the promoter are final and no correspondence will be entered to.
11.By entering, the prize winner agrees to be available for reasonable publicity purposes by Amazon without compensation, including but not limited to photographs & interviews in the 3 month period following the prize notification. Entrants acknowledge that their details may be published in various media.
12.The Prize of a $500 Shopping Experience must be used in one transaction and needs to be redeemed before 31st December 2014. Where the full $500 is not spent, no change will be given and the prize will be deemed to be redeemed in full.
